Call the toll-free number 800 373-6950, or logon to www.ubshealth.com and click on the “patient portal” button. Complete the registration process, and request an appointment, or call for a triage session with a nurse.
I can’t find my membership card?
Call in to our Customer Service 1(866) 228-5111 and request a new one be sent to you.
What is included in my membership?
Login to www.ubshealth.com and click on “Account”. Your account details are in your profile.
How do I add a dependent to my account?
Login to www.ubshealth.com and choose a plan for your dependent.
How can I update my medical information?
In the portal, you can click on your medical records icon, and add in the desired category
When is my next payment?
You are billed monthly. Usually on or about the 1st of the month.
How much will it cost to see a doctor?
Any encounter with a clinician is $45.00 unless you subscribe to a wellness plan.
Is this plan considered insurance?
No, this is an adjunct to insurance.
Can I get a prescription today?
Those decisions are made by the clinician.
Does the $14.95 include my whole family?
Up to 4, any additional will charge .50c per person
I am registered but my family member is sick can they get nurse advice too?
Yes. You must register them in order for us to keep accurate medical records.
Is my whole family included in a family plan?
Up to 4 Members
